In a recent report, experts have highlighted potential risks faced by ethnic minority newborns in terms of their health assessments within the National Health Service (NHS). One particular test, known as the Apgar test, which evaluates a baby’s overall health within the first 10 minutes after birth, has been deemed unfit for purpose for black, Asian, and ethnic minority infants. Developed in 1952, the Apgar test measures the baby’s heart rate, reflexes, muscle tone, breathing, and skin color to determine their overall well-being. However, the test relies on visual observations, often referring to colors like pink, blue, or pale to describe the baby’s skin tone, without considering potential variations among ethnic minority newborns.

According to a review conducted by experts at Sheffield Hallam University for the NHS Race and Health Observatory, the lack of differentiation in guidelines for healthcare workers can lead to ineffective assessments of minority ethnic babies. Furthermore, the report states that healthcare workers and parents are not adequately trained to identify conditions such as jaundice or cyanosis in black, Asian, and ethnic minority babies.

To address these concerns, the report suggests the use of devices that can more accurately detect conditions like jaundice and cyanosis, rather than solely relying on visual examinations. This would help overcome potential biases in assessments and improve outcomes for diverse communities.

Dr. Habib Naqvi, the chief executive of the NHS Race and Health Observatory, emphasized the need for addressing the limitations in visual examinations, including the Apgar scores, which may disadvantage babies with darker skin tones. The observatory aims to provide practical solutions to challenges related to patient safety, focusing on reducing disparities in healthcare interventions and assessments.

Hora Soltani, a professor of maternal and infant health at Sheffield Hallam University, highlighted the importance of listening to parents’ concerns and ensuring policies and education for healthcare professionals reflect the impact of various skin color tones in neonatal assessments.

In response to these concerns, the Department of Health and Social Care emphasized their commitment to high-quality maternity care for everyone. They highlighted the publication of guidance by NHS England for local maternity systems, which aims to reduce disparities for ethnic minorities and those in deprived areas. Additionally, the Maternity Disparities Taskforce has been established, bringing together experts from across the healthcare system to explore evidence-based interventions addressing maternal disparities.

This review underscores the need for inclusive and culturally sensitive healthcare practices. By considering the specific needs of ethnic minority newborns, healthcare professionals can ensure accurate assessments and timely diagnoses, ultimately leading to improved outcomes for all babies. It is vital to prioritize patient safety and work toward reducing healthcare disparities among diverse communities.
