Hundreds of Errors Found in Radiologist’s Work at Letterkenny Hospital, Major Investigation Underway, Ireland

A major investigation has been launched at Letterkenny University Hospital in Ireland following the discovery of numerous errors in the work of a locum radiologist. The review was initiated after it was found that one third of the locum’s work contained mistakes.

The contract of the locum consultant was terminated within 10 days due to concerns raised about the quality of their work, according to the Saolta University Health Care group. Following the departure of the locum, a senior staff member brought attention to the hospital’s management, highlighting the initial failure to review the scans. Internal correspondence seen by The Irish Times revealed that the staff member complained about this issue.

Senior hospital staff and local general practitioners had previously called for an external inquiry into Letterkenny Hospital, citing concerns over its stability. The hospital is currently facing significant challenges in recruiting and retaining senior medical staff, contributing to its ongoing struggles.

In response to the review of the locum’s scans, a spokesman from Saolta stated that a comprehensive evaluation of the clinician’s work is underway to ensure patient safety. It is important to note that the locum consultant was only employed at the hospital for seven days.

The hospital’s quality and patient safety service has implemented a clear process for addressing concerns regarding clinical care, which is presently being followed. The spokesman also added that in the event of any misdiagnosis cases coming to light, patients will be contacted with transparent disclosure, and suitable arrangements will be made for follow-up care in accordance with national policy.

Following the termination of the locum’s contract, a senior consultant at Letterkenny Hospital reached out to the hospital and Saolta management to request a review of the individual’s work. The consultant claimed that they received no response. Subsequently, the consultant took it upon themselves to review the computed tomography (CT) scans conducted by the locum, finding an error rate of 37 percent.

According to the consultant, they have yet to receive a reply from hospital management despite bringing this matter to their attention in October. The consultant expressed a lack of confidence in the department being adequately led by consultants and stressed the need for an external review of the radiology department’s management.

In light of the ongoing investigation and the concerns raised, it is crucial to maintain a balanced perspective and consider the various viewpoints surrounding the issue. The focus remains on addressing any errors made, ensuring patient safety, and implementing measures to improve the overall quality of care provided by Letterkenny University Hospital.
