Fatal Surge in Fleaborne Typhus Cases in Los Angeles County Shocks Experts
Experts are expressing shock and concern over a recent surge in fleaborne typhus cases in Los Angeles County. According to research published in the U.S. Centers for Disease Control and Prevention Morbidity and Mortality Weekly Report, there were three reported deaths associated with fleaborne typhus in 2022. This marks a significant increase from previous years, as there were only 31 cases reported in 2010.
The study, led by Jemma Alarcón, M.D., of the CDC in Atlanta, examined the clinical presentation, illness course, and diagnostic methods used in these fleaborne typhus-associated deaths. The researchers discovered severe manifestations of the disease in these cases, including hemophagocytic lymphohistiocytosis, myocarditis, and septic shock with disseminated intravascular coagulation. It is worth noting that all three patients had underlying health conditions that may have contributed to the severity of their illness.
One of the challenges in diagnosing fleaborne typhus is that testing for Rickettsia typhi, the bacterium that causes the disease, during early illness might yield low or undetectable antibody titers. Additionally, waiting for convalescent titers for confirmation of diagnosis can result in delays in treatment. To address this, the authors recommend that healthcare providers start treatment with doxycycline as soon as fleaborne typhus is suspected.
